Job summary

Family Service League Inc. in Bay Shore, NY is seeking a part-time Long Term Care Coordinator for our Ombudsman program.

The role focuses on empowering, educating, and advocating for residents in licensed facilities across Suffolk County. You will mentor volunteers, visit facilities lacking Ombudsman coverage, and ensure compliance with state regulations while maintaining clear reporting and documentation by deadlines.

Qualifications

  • Bachelor’s Degree in human services, social work or related field required.
  • NYS Certified Ombudsman certification required - training provided and must be completed within 6 months.
  • Minimum 3 years’ experience with older adults and disabled individuals preferred.
  • Strong interpersonal and communication skills.
  • Understanding of aging, health care and long-term care issues preferred.
  • Excellent organizational skills and ability to meet deadlines.
  • Self-starter, able to work independently and as part of a team.
  • Excellent verbal and written communication and presentation skills.
  • Computer proficiency, including Microsoft Office.
  • Valid New York State Driver’s License required.

Responsibilities

  • Visit long term care facilities without an Ombudsman to ensure residents have access to advocacy.
  • Travel across Suffolk County to provide coverage and mentor volunteers.
  • Escalate critical issues to Program Coordinator when needed.
  • Assist residents and families to form councils and advocate for needs.
  • Complete reports and documentation accurately and on time.
  • Attend staff meetings, trainings, and program sessions.
  • Assist with certification training of new Ombudsman volunteers.
  • Ensure compliance with policies and regulatory requirements including NYS DOH and OSSLT.
  • Engage with service providers, advocacy groups, and regulatory agencies.
  • Provide resources and referrals to residents and families navigating long-term care.
