Duties & Responsibilities
Purpose of Position

Under supervision of the Social Work Director or designee, plans, designs and evaluates programs and systems (or subdivisions thereof) relating to social work and discharge planning services, and implements these by appropriate analytical, liaison, consultative and research functions.

Provides support to oversee day to day operations of the social work department as well as assist the director (or designee) in ensuring timely coordination, and delivery of patient and resident related activities and services. Duties may also include facilitating projects and other tasks assigned to improve service delivery and discharge planning.

Essential Duties And Responsibilities
  • Responsible for initiating and coordinating all LTACH ALOC and SNF referrals to various community base facilities:
    • Consults with social workers, personnel in other segments of the Corporation involved in the direct patient care, financial, operational and/or professional components of health care program plans.
    • Consults with medical and professional staff of other departments and staff from associated health care fields to plan and coordinate joint patient and management objectives. Acts as a liaison between the Department of Medical Services and other administrative divisions.
    • Liaisons and coordinates activities with operational health facilities personnel, representatives of the community and involved governmental or health care agencies.
    • Works as a member of an interdisciplinary team assisting in making referrals to City agencies, social service agencies, community organizations and other organizations that provide health and social services.
    • As required, consults with medical, nursing and other staff involved in treatment to clarify ambiguities and secure documentation of the records and to secure necessary and timely certification and verification.
    • Provides technical/professional assistance within the social work department i.e. distributing, gathering, and submitting discharge notices, providing limited direct patient/resident interaction to support social worker
  • Participates in developing strategies and alternative approaches for improvement of discharge planning and delivery of patient/resident care within the social work department.
  • Provides administrative supervision for a health service program.
  • Participates in designing evaluative measures and procedures by which program efficiency in delivery of patient/resident care can be audited and participates in the evaluation audits.
  • May supervise studies related to the service/program and analyzes modifications and development of systems and procedures to improve departmental operations. Makes recommendations and implements.
  • Supervises the office administration and office record keeping activities of the department.
  • Assists in planning and development of intrafacility and community materials, preparation of interpretive bulletins and procedures, etc.
  • Represents the facility/department at community activities related to agency or program concerns.
  • Participates in planning, organizing, and implementing overall community liaison programs with nursing facilities, home health agencies and other appropriate community groups.
  • Assists in the preparation of program guidelines, designs or protocols, and delineation of problems relating to their implementation.
  • Supervises the preparation and publication of operational manuals and procedures.
  • Plans and conducts departmental analysis on data and metrics such as:
    • Keeps records related to programs, activities, objectives, etc.
    • Feasibility Studies and Resource Analysis;
    • Content and Standards for the professional components of health care projects;
    • Analyst of socio-economic and other demographic data;
    • Public policy and Legal Restraints; and,
    • Staffing patterns.
  • Participates in defining and developing data needed for planning and reporting. Converts data into quantification measures for utilization i.e. Ratios and Patterns, Manpower and Staffing needs, Pricing and Costs Formula and other analysis pertinent to social work and discharge planning.
  • Performs other related tasks.
Minimum Qualifications

Master’s degree from an accredited college or university in Public Health, Public Administration, Business Administration, Social Work, Psychology or Rehabilitation Counseling; and one (1) year of full-time experience in medically-oriented health care and medical support systems environment; or 2. Bachelor’s degree from an accredited college or university in disciplines listed in “1” above; and two (2) years of full-time experience in medically-oriented health care and medical support systems environment; or 3. A satisfactory equivalent combination of education, training and/or experience, in which each year of relevant training and/or experience can be substituted for 30 semester credits from an accredited college or university. However, all applicants must at least have a Bachelor’s degree.
