Job summary

Maxim Healthcare is hiring in Santa Monica, CA for roles that support clients in living independently at home and in the community. The position emphasizes coordination with families, scheduling medical appointments, and documenting progress.

• Current BLS or CPR/First Aid certification.

Responsibilities

• Coordinate and facilitate services that support clients’ ability to live independently in their family home.
• Assist clients and families in navigating and accessing community resources and generic services.
• Provide support and training in daily living skills, both in-home and in the community.
• Communicate regularly with families to ensure alignment and progress.
• Help schedule and coordinate medical and other essential appointments.
• Supervise clients during community outings as needed.
• Maintain accurate and timely documentation, including quarterly progress reports.
• Uphold strict confidentiality and report changes in client status per care plan guidelines.

What You’ll Do
