Job summary
Okanogan County Community Action Council is seeking a Coordinated Entry & FCS Lead to advance foundational community supports and homeless services across Okanogan County. The role combines case management with coordinated entry leadership, coordinating with Medicaid providers and CHG partners to connect clients with housing and employment supports.
The position requires five years in case management, a bachelor’s degree in a related field, and bilingual English/Spanish.
Qualifications
- Minimum five years of experience in case management or related field.
- Bachelor's degree in social work, human services, public health, psychology, or related field.
- Bilingual in English and Spanish preferred.
- Proficient in Word, Excel, and Outlook; time-tracking and purchasing software.
Responsibilities
- Provide FCS case management for Medicaid-enrolled clients.
- Coordinate and monitor Supported Employment and Housing services.
- Develop individualized service plans with clients and support networks.
- Maintain required state documentation and data systems.
- Coordinate CE referrals and connect clients to housing resources.
- Market FCS services to providers to grow enrollment.
