Delta Homecare is seeking an RN to provide in-home nursing care and supervises caregivers in patient homes and communities. You will conduct assessments, admissions, and care-plans, while coordinating with the office and healthcare team.

The role requires current Georgia RN licensure, at least two years of general nursing experience (home care, medical, surgical, or critical care), and a valid driver's license with a reliable vehicle.

Qualifications

• Must be a graduate of an accredited nursing program.
• Licensed RN in Georgia.
• Minimum 2 years general nursing experience in home care or acute settings.
• Maintains professional standards and personal health.
• Valid driver's license with insured vehicle.

Responsabilités

• Provide in-home skilled nursing care and monitor patient conditions.
• Perform supervisory visits with caregivers via phone or in-home as needed.
• Conduct admissions assessments, consent forms, and care plan creation.
• Coordinate with office, caregivers, and other healthcare professionals.
