Job summary

Precision-Healthcare in Unity, NH seeks a qualified Physical Therapist (PT) for a contract assignment. The PT will perform evaluations, develop treatment plans, and deliver comprehensive therapy to improve mobility and function.

Work with an interdisciplinary team to ensure high-quality patient care and discharge planning. Candidates should hold an NH PT license, be a graduate of an accredited PT program, and ideally have CPR/BLS certification.

Qualifications

• NH PT license required.
• Graduate of an accredited Physical Therapy program.
• Current CPR/BLS certification preferred.
• Experience in skilled nursing/rehabilitation/long-term care preferred.

Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide therapeutic exercises, gait training, balance activities, and functional rehabilitation.
• Monitor patient progress and adjust treatment plans as needed.
• Document evaluations, treatments, progress, and discharge planning accurately.
• Collaborate with physicians, nursing staff, and other therapists.
