Job summary

Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Physical Therapist to join a great SNF/LTC for a contract assignment in Indiana. The role requires delivering patient-centered PT care in alignment with physician orders and organizational goals.

The position is a contract, day-shift role with responsibilities including developing care plans, performing PT interventions, and collaborating with an interdisciplinary team to meet patient goals.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist (PT) or actively pursuing licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet PT goals.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
