Job summary

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment! This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or actively in the licensure process.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assess, plan, evaluate and modify care to meet PT goals.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

Responsibilities
