Job summary

Responsibilities include developing a plan of care, delivering skilled PT services per physician orders, assessing and adjusting care, and collaborating with all disciplines to meet patient goals.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in the process of applying for licensure.
• Current CPR certification.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
