Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Physical Therapist (PT) for a contract assignment in a SNF/LTC setting. The role requires responsibility and accountability for integrating the organization's vision, values, and goals into daily practice.

Qualified candidates have graduated from an accredited program, possess a current PT license or are in process, CPR certification, and a collaborative approach to patient care with physician orders.

Formación

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process of licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

Responsabilidades

• Develops and implements individualized PT plans of care.
• Delivers skilled PT interventions per physician orders.
• Collaborates with interdisciplinary team to optimize patient outcomes.
• Monitors progress and adjusts treatment plans accordingly.
• Ensures documentation aligns with clinical goals.
