Centra Healthcare Solutions is seeking an experienced or newly graduated Physical Therapist for a contract assignment in a skilled nursing facility (SNF/LTC). You will develop care plans and deliver skilled PT services, working with the care team to meet patient goals.

Candidates must graduate from an accredited program, hold or be pursuing state PT licensure, and maintain CPR certification. This contract offers competitive pay and the chance to gain diverse clinical experience nationwide.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process.
• Current CPR certification.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.
• Collaborate with disciplines to plan and evaluate team goals.
