Job summary

Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Occupational Therapist to join a SNF/LTC team on a contract assignment. This contract position offers day schedule and accountability to the organization's mission and goals.

The OT will provide skilled services per physician orders, assess needs, plan and modify care, and collaborate with the interdisciplinary team to achieve patient outcomes.

Qualifications

• Must have graduated from an accredited OT program.
• Current state license as an Occupational Therapist or in the process of licensure.
• Current CPR certification.

Responsibilities

• Provide skilled occupational therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate and modify care to meet OT goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
