Job summary

FullShift Staffing, LLC seeks an LVN/LPN to provide nursing care in a Colorado long-term care setting. You will follow physician orders, document patient changes, and support clinical operations in collaboration with a multi-disciplinary team.

Responsibilities include charting per established procedures, narcotics counting, and reporting, as well as rounds with physicians as needed. Active LVN/LPN licensure and BLS/CPR certification are required or willingness to obtain.

Qualifications

• Experience in long-term care settings.
• Proficient in PointClickCare charting.
• Active LVN/LPN license in the practicing state.
• BLS/CPR certification or willingness to obtain.

Responsibilities

• Follow established procedures for charting and incident reporting.
• Adhere to narcotic counting, pharmacy orders, and restraint reduction protocols.
• Document changes in patient conditions (nutrition, infections, skin issues, weight loss, etc.).
• Utilize the Quality Life Manual to identify, place, and monitor patients in appropriate programs.
• Document and notify departments of room changes, LOA, admissions, or discharges.
• Rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience


• Point click care charting experience


• Current LVN/LPN license in practicing state.


• Current BLS/CPR certification or willingness to acquire one.


• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.


• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.


• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.


• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.


• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.


• Makes rounds with physicians when necessary.
