Job summary

Deblin Health Concepts, part of the SUN Behavioral Health network, is seeking a Case Manager (Contract) to join the treatment team in the Greater Houston Area. You will conduct assessments, develop and monitor treatment plans, and coordinate care with therapists and providers across school-based settings.

The role requires a Master's in a Human Services field, at least two years of case management experience, CPR certification, and the ability to travel locally between campuses and community

CASE MANAGER (CONTRACT)LocationGreater Houston AreaSummaryThe Case Manager is a key member of the treatment team responsible for providing case management services to individuals with Level of Care 3 and/or 4 needs. This role conducts assessments, coordinates care, develops and monitors treatment plans, and connects clients with community resources to support stability and well-being. The position also provides individual and group case management services within school-based settings, supporting students across elementary, middle, and high school campuses.
