Job summary

Deblin Health Concepts in the Greater Houston Area seeks a Contract Case Manager to support LOC 3/4 clients, including school-based services, with local travel between campuses.

## DescriptionCASE MANAGER (CONTRACT)# Location# Greater Houston Area# SummaryThe Case Manager is a key member of the treatment team responsible for providing case management services to individuals with Level of Care 3 and/or 4 needs. This role conducts assessments, coordinates care, develops and monitors treatment plans, and connects clients with community resources to support stability and well-being. The position also provides individual and group case management services within school-based settings, supporting students across elementary, middle, and high school campuses.
