Responsibilities

• Accurately assess, differentially diagnose, and identify deficits in a patient’s ability to move and perform functional activities.
• Develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability.
• Perform patient evaluations: history, systems review, tests and measures to obtain data.
• Establish a care plan and select treatments addressing impairments per physician orders.
• Reassess patient progress and adjust treatment as needed throughout care.
• Initiate and manage discharge planning from first visit through treatment.
• Supervise/delegate to staff and students per state laws and policy.
• Ensure clear, timely documentation per policy and regulatory requirements.
• Educate patient, family, caregiver on needs and safety.
• Provide safe environment and adhere to safety standards.

Job Description:
