Position Requirements:

• Registered Nurse: 2 years - Required
• Recent/Current Home Health Nursingexperience: 1 year (within the last 3 years) - Required
• H ome Care Home Base (HCHB)charting experience - Required

Job Description:

• Provide and coordinate care forassigned patients in comfort of their homes.
• Develop and implement nursingcare plans including Start of Care and admissions as assigned.
• Support patient recovery andability to stay at home - enabling them to spend quality time with theirloved ones and doing the activities they enjoy
• Empower patients and families toreach healthcare goals by educating them on disease management
• Case management and provideclinical oversight to interdisciplinary
• Provide nursing care and serviceswithin the standards and scope of nursing practice as defined within thestate Nurse Practice Act regulations, policies, procedures, and protocols,as well as facility and corporate policies and procedures.
• Ensure physicians’ orders arelegible if taken verbally and transcribed appropriately per policy andprocedure.
• Ensure patients’ status isassessed on an ongoing basis and pertinent information is gathered anddocumented.
• Notify physicians of significantchanges in status
• Document and complete chartingfor all patient visits within 24 hours.
• Territories to cover:Primarily Wausau. Depending on Volume, there would be additional visits to cover in Stevens Point/Plover, Marshfield, Plover.
