Qualifications

• RN with 2 years of home health nursing experience required.
• 1 year of recent/home health nursing experience within last 3 years required.
• OASIS experience required.
• HCHB charting experience required.
• Must have valid driver’s license and personal auto insurance.

Responsibilities

• Provide and coordinate care for assigned patients in their homes.
• Develop and implement nursing care plans for Start of Care and admissions.
• Support patient recovery and ability to stay at home with loved ones.
• Educate patients and families to reach healthcare goals and disease management.
• Perform case management and clinical oversight with interdisciplinary team.
• Provide nursing care and services per state nursing practice acts and policies.
• Ensure physicians' orders are legible and properly transcribed.
• Assess patient status continuously and document pertinent information.
• Notify physicians of significant changes in status.
• Document and complete charting for all visits within 24 hours.
• Territories to cover Spokane County and possibly lower Stevens County.

• Notify physicians of significantchanges in status
