Job summary

Sage GroupHealthCare Staffing is seeking a Home Health Registered Nurse for a 13-week travel/contract position in Sturgeon Bay, WI. We are accepting local and traveler candidates to provide in-home professional nursing care.

Responsibilities include planning and coordinating care for assigned patients, implementing Start of Care and admissions, documenting visits within 24 hours, and meeting OASIS/HCHB requirements.

Qualifications

• RN with 2 years of experience in home health or equivalent.
• OASIS experience required.
• Home Care Home Base (HCHB) charting experience required.
• Must have valid driver’s license and personal auto insurance.

• Provide and coordinate care for assigned patients in the home setting.
• Develop and implement nursing care plans including Start of Care and admissions.

Job Description:

• Provide and coordinate care forassigned patients in comfort of their homes.
• Develop and implement nursingcare plans including Start of Care and admissions as assigned.
• Support patient recovery andability to stay at home - enabling them to spend quality time with theirloved ones and doing the activities they enjoy
• Empower patients and families toreach healthcare goals by educating them on disease management
• Case management and provideclinical oversight to interdisciplinary
• Provide nursing care and serviceswithin the standards and scope of nursing practice as defined within thestate Nurse Practice Act regulations, policies, procedures, and protocols,as well as facility and corporate policies and procedures.
• Ensure physicians’ orders arelegible if taken verbally and transcribed appropriately per policy andprocedure.
• Ensure patients’ status isassessed on an ongoing basis and pertinent information is gathered anddocumented.
