Job summary

Sage GroupHealthCare Staffing is seeking a Home Health LPN for a travel/contract position in Spokane Valley, WA. Start ASAP. Full-time, travel, contract, or temporary assignment for 13 weeks. Mileage reimbursed; travel/housing stipend available for travelers.

Provide skilled nursing care under RN supervision, monitor conditions, administer medications, educate patients and families, and complete documentation per Medicare guidelines.

Qualifications

• LPN experience: 1 - 2 years - Required
• Recent Home Health nursing experience: 1 year required, within last 3 years
• Home Base charting (HCHB) - Preferred

Responsibilities

• Provide skilled nursing care as ordered by the physician under RN supervision.
• Observe and monitor patient conditions and notify RN case manager of changes.
• Administer medications as prescribed under RN supervision and educate patients to reduce readmissions.
• Educate patient and family using teach-back methods for understanding.
• Complete clinical documentation per agency protocol and Medicare guidelines.
• Adhere to agency policies and communicate scheduling changes as needed.
• Attend IDT Team meetings.

Pay Rate:Range $2000 to $2080 Weekly Gross pay based on a 40-hr work week ( Includes Non-taxabletravel/housing and M&I stipend available for qualified/eligible travelers)

MileageReimbursement: $0.49/mile for distancecovered between patient home appointments

Unit: Home Health

Shift timings: 8 Hours; Days, 8:00 am - 5:00 pm

Position Requirements:

• LPN experience: 1 - 2 years - Required
  
• Recent/Current Home Health Nursingexperience: 1 year required and preferably within the last 3 years
  
• Home Care Home Base (HCHB)charting - Preferred
  
• Must have valid Driver’s Licenseand Personal Auto Insurance

Education, License, and Certification Requirements:

Job Description:

• Administer medication as prescribed by the physician under RN'ssupervision and help decrease re-hospitalizations by educating the patient onpreventative measures and making good decisions.
