Job summary

Sage GroupHealthCare Staffing is actively seeking a Registered Nurse Home Health for a 13-week travel/contract assignment in Lafayette, IN. This role covers Lafayette and surrounding areas, with 40 hours per week and a mix of travel and temporary duties.

Position requires AS N/BSN, RN license, OASIS and HCHB experience, and Covid vaccination. Pay ranges weekly and includes travel/housing stipends for eligible travelers.

Qualifications

• RN with 2+ years of experience in nursing.
• Recent/current home health experience preferred.
• OASIS and HCHB charting experience required or recent.

Responsibilities

• Provide and coordinate care for Home Health patients.
• Develop and implement nursing care plans for assigned patients.
• Educate patients and families on disease management.
• Deliver Home Health nursing care per nursing act and policies.
• Ensure legible physicians' orders and proper transcription.
• Monitor patient status and document findings timely.
• Notify physicians of significant changes in status.

Shift: 8 Hours; Days, 8.00a - 5.00p

Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, Productivity expectation - 32 points per week. On call: 1 weeknight shift per month and 1 weekend shift for month

Job Description:

• Provide andcoordinate care for assigned Home Health patients

• Provide Home Health nursingcare and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
