Job summary

Spectrum Health Systems, Inc. in Weymouth, MA is seeking a Continuing Care Coordinator for the CSS program. The role includes coordinating recovery services for clients re-entering the community and providing aftercare counseling on campus.

Qualifications

• Strong documentation, organizational, and communication skills
• Must have basic ability in use of computers to be able make entries and utilize electronic health records
• Minimum of a high school diploma or equivalent

Responsibilities

• Coordinate continuing recovery services for clients returning to the community after completion of the CSS program.
• Perform case management activities regarding housing, job searches, and establishment of medical and social services and education.
• Coordinate continuing care recovery referrals with community providers and schedule initial appointments.
• Work closely with Clinical staff to ensure smooth transition from residential care to continuing care program.
• Collect data and program success rates as directed.

Schedule: Sunday-Thursday, 8:00am-4:30pm

Pay Rate: $22.00-$25.00/hour

Continuing Care Coordinator

The Continuing Care Coordinator is responsible for:

• The coordination of continuing recovery services for clients returning to the community after completion of the Clinical Stabilization Services program and for providing aftercare counseling services to clients on campus.
• Performing case management activities based on clients' continuing care plans regarding initial housing, job searches, and establishment of medical services, social services, education, support services, and services from other community providers.
• Coordinating continuing care recovery referrals by developing positive relationships with community providers and including scheduling of initial appointments.
• Working closely with the Clinical staff in the Clinical Stabilization Services program to ensure the clients transition smoothly from the residential portion of treatment to the continuing care program.
• Collecting data and program success rates as directed.
