This role offers a pay range of $25.82/hr - $36.50/hr, based on skills and experience. Immediate openings are available for Service Coordinators working with diverse age groups, including a critical need in the School Age group. Bilingual positions are also available in Antelope Valley, Santa Clarita Valley, and Chatsworth locations.
Provides information, advocacy, and service coordination for consumers and families. Responsibilities include developing and monitoring individual program plans, coordinating services, documenting in accordance with regulations, advocating for individuals, attending interdisciplinary meetings, and addressing barriers to service delivery with culturally sensitive strategies. Rotation of officer-of-the-day duties and emergency on-call services are also part of the role.
Community-based position requiring meetings with individuals and families in various settings. Must be comfortable working independently, with frequent virtual and out-of-office meetings. A valid California Driver’s License and reliable transportation are required.
