Responsibilities

• Coordinates pharmaceutical services within a long term care facility in collaboration with facility leadership and staff.
• Guides development and evaluation of pharmaceutical services procedures with facility leadership.
• Addresses pharmacy-related concerns to protect resident care, regulatory compliance and quality of life.
• Provides medication-related education and quarterly reports on drug utilization and cost savings.
• Attends Quality Assurance Committee meetings and reports on therapeutic interchange and staff performance.
• Assists in developing policies to ensure compliance with federal and state regulations for drug handling and administration.
• Interacts with auditors and outside agencies as needed.
• Completes mandatory regulatory training and performs other duties as assigned.

Job Summary: Responsible for the implementation and adherence to Health Direct’s Standard of Practice as it relates to the provision of the highest quality health care service to our customers
Job Duties

• Develops and identifies best practices working with advanced practitioners, professional managers of pharmaceutical services, and other relevant healthcare professionals to achieve successful outcomes. They will be acknowledged sources of expertise, and will contribute to the development of service strategies, which will drive change across health and social care.
• Coordinates pharmaceutical services within a long term care facility (facility), in collaboration with facility leadership and staff
• Guides development and evaluation of pharmaceutical services procedures within the facility, in collaboration with facility leadership and staff
• Assists the facility identify, evaluate, and address/resolve pharmacy-related concerns and issues that affect resident care, medical care, quality of life, and/or regulatory compliance
• Spend an appropriate amount of time on each resident’s review as defined by their clinical needs
• Provide quarterly reports reflecting facility-level drug utilization and clinical management trends, with corresponding recommendations to improve clinical care delivered and potential savings using preferred drug management and formulary compliance
• Provide medication-related education
• Attend quarterly facility Quality Assurance Committee meeting and provide written reports regarding the status of the facility’s pharmaceutical services, therapeutic interchange cost savings report and staff performance
• Assist the facility in developing and implementing policies and procedures to ensure compliance with federal and state regulations related to ordering, storage, handling, labeling, destruction and administration of drugs and biologicals
• Assist the facility in establishing a procedure to perform MRR on residents who are anticipated to stay in the facility less than 30 days, or who experience an “acute” change of condition, as identified by facility staff
• Coordinate or perform review of, using representative sampling, medication storage and labeling in common storage areas (medication carts, medication refrigerators, medication rooms);
• Coordinate or perform review of medications stored in facility Emergency Drug Supply which would include review for outdated medications and a continual review of contents with facility management for appropriateness
• Provide assistance (by telephone if necessary) with issues or questions that arise either prior to or on the day of any survey of the facility
• Reports dispensing incidents in a timely manner, in accordance with company guidelines
• Follows all State and Federal guidelines governing the practice of Pharmacy
• Maintains and develops professional relationships with doctors and others in the health service professions
• Maintains an awareness of developments in the Pharmacy and Health Care Services fields that relate to job responsibilities and integrates them into own practice
• Maintains confidentiality regarding all patient information
• Facilitates communication between professional staff and pharmacy personnel
• Interface with various auditors and outside agencies, as needed
• Responsible for completing all mandatory and regulatory training programs
• Perform other duties as assigned
