Job summary

Priority Dispatch Corp. seeks a licensed pharmacist to implement Health Direct’s Standards of Practice within long-term care settings, ensuring the highest quality pharmaceutical care and strict regulatory compliance.

The role coordinates pharmaceutical services with facility leadership, guides policy development, addresses drug-related concerns, and provides education. It supports quarterly reports, QA activities, and collaboration with doctors and facility staff to optimize resident care.

Qualifications

• Possess and maintain a valid State License and current registration to practice Pharmacy.
• BS or PharmD degree in Pharmacy.

Responsibilities

• Implements Health Direct’s Standard of Practice to provide high-quality pharmaceutical care.
• Coordinates pharmaceutical services within long-term care facilities with facility leadership.
• Develops and evaluates procedures for pharmaceutical services in collaboration with staff.
• Addresses pharmacy-related concerns affecting resident care and regulatory compliance.
• Provides medication-related education and quarterly drug utilization reports.
• Attends Quality Assurance Committee meetings and prepares written reports.
• Ensures compliance with federal and state regulations on drug handling and administration.
• Fosters professional relationships with doctors and health care staff.
• Maintains confidentiality of patient information and interfaces with auditors as needed.

Job Summary: Responsible for the implementation and adherence to Health Direct’s Standard of Practice as it relates to the provision of the highest quality health care service to our customersJob Duties:Develops and identifies best practices working with advanced practitioners, professional managers of pharmaceutical services, and other relevant healthcare professionals to achieve successful outcomes. They will be acknowledged sources of expertise, and will contribute to the development of service strategies, which will drive change across health and social care.Coordinates pharmaceutical services within a long term care facility (facility), in collaboration with facility leadership and staffGuides development and evaluation of pharmaceutical services procedures within the facility, in collaboration with facility leadership and staffAssists the facility identify, evaluate, and address/resolve pharmacy-related concerns and issues that affect resident care, medical care, quality of life, and/or regulatory complianceSpend an appropriate amount of time on each resident’s review as defined by their clinical needsProvide quarterly reports reflecting facility-level drug utilization and clinical management trends, with corresponding recommendations to improve clinical care delivered and potential savings using preferred drug management and formulary complianceProvide medication-related educationAttend quarterly facility Quality Assurance Committee meeting and provide written reports regarding the status of the facility’s pharmaceutical services, therapeutic interchange cost savings report and staff performanceAssist the facility in developing and implementing policies and procedures to ensure compliance with federal and state regulations related to ordering, storage, handling, labeling, destruction and administration of drugs and biologicalsAssist the facility in establishing a procedure to perform MRR on residents who are anticipated to stay in the facility less than 30 days, or who experience an “acute” change of condition, as identified by facility staffCoordinate or perform review of, using representative sampling, medication storage and labeling in common storage areas (medication carts, medication refrigerators, medication rooms);Coordinate or perform review of medications stored in facility Emergency Drug Supply which would include review for outdated medications and a continual review of contents with facility management for appropriatenessProvide assistance (by telephone if necessary) with issues or questions that arise either prior to or on the day of any survey of the facilityReports dispensing incidents in a timely manner, in accordance with company guidelinesFollows all State and Federal guidelines governing the practice of PharmacyMaintains and develops professional relationships with doctors and others in the health service professionsMaintains an awareness of developments in the Pharmacy and Health Care Services fields that relate to job responsibilities and integrates them into own practiceMaintains confidentiality regarding all patient informationFacilitates communication between professional staff and pharmacy personnelInterface with various auditors and outside agencies, as neededResponsible for completing all mandatory and regulatory training programsPerform other duties as assignedJob Requirements:Posses and maintain a valid State License and current registration to practice PharmacyAbility to stand/walk throughout majority of shiftSuccessful completion of a pre-employment and/or random drug screening testEducation:Minimum: BS or Pharm D degree in Pharmacy
