Job summary

Kalamazoo Long Term Care Pharmacy is seeking a part-time Consultant Pharmacist to work on-site at the PACE center in Alpena, MI. You will lead medication management as the on-site pharmacy expert within the center's care team.

Responsibilities include comprehensive med reviews, collaboration with the IDT, and ensuring CMS PACE compliance. Must hold PharmD and Michigan licensure eligibility, with favorable geriatrics and geriatrics/pharmacy experience.

Qualifications

• PharmD from an ACPE-accredited program.
• Active, unrestricted Michigan pharmacist license or eligibility to obtain one.

Responsibilities

• Perform comprehensive medication reviews and reconciliations for PACE participants at enrollment, during care transitions, and after hospital stays.
• Join care planning meetings and reassessments with the interdisciplinary team (IDT).
• Collaborate with physicians, nurse practitioners and nursing staff to resolve medication issues and support evidence-based prescribing.
• Identify polypharmacy and high-risk medications and opportunities to deprescribe.
• Act as liaison between the center and the pharmacy, resolving refill, delivery and MAR issues in real time.
• Oversee on-site medication storage, inventory and emergency supplies.
• Educate staff, participants and caregivers about medications, adherence and ordering processes.
• Support formulary management, quality improvement and CMS PACE and Michigan Board of Pharmacy compliance.

Part time (one day per week) On site $61-$67 per hour

Kalamazoo Long Term Care Pharmacy is looking for a part-time Consultant Pharmacist to work on site at the PACE center we support in Alpena. PACE (Program of All-Inclusive Care for the Elderly) helps older adults stay safely in their homes and communities. Medication management is central to that work.

In this role you'll be the on-site pharmacy expert and a core member of the center's care team. You'll have the time and the relationships to make a real difference in participants' therapy. What you bring

• Knowledge of CMS PACE and long-term care pharmacy regulations
• Strong communication skills, problem-solving ability and attention to detail
• The confidence to work independently on site while collaborating with an interdisciplinary team

Nice to have

• Long-term care, PACE or geriatric pharmacy experience
• Board Certified Geriatric Pharmacist (BCGP)
• Experience with Framework or MediProcity
