Job summary

Kalamazoo Long Term Care Pharmacy is seeking a Part-Time Consultant Pharmacist to work on site at the PACE center in Alpena, Michigan. One day per week, on-site role focused on medication management for older adults.

You will conduct comprehensive medication reviews, participate in care planning, collaborate with physicians and nursing staff, resolve medication issues, and support CMS PACE requirements. This is a meaningful, team-based clinical role in a community setting.

Qualifications

• PharmD from an ACPE-accredited program.
• Active, unrestricted Michigan pharmacist license, or eligibility to get one before you start.
• Knowledge of CMS PACE and long-term care pharmacy regulations.
• Strong communication skills, problem-solving ability and attention to detail.
• The confidence to work independently on site while collaborating with an interdisciplinary team.

Responsibilities

• Perform comprehensive medication reviews and reconciliations for PACE participants at enrollment, during transitions and after hospital stays.
• Join care planning meetings and reassessments with the interdisciplinary team (IDT).
• Collaborate with physicians, nurse practitioners and nursing staff to resolve medication issues and support evidence-based prescribing.
• Identify polypharmacy, high-risk medications and opportunities to deprescribe.
• Act as liaison between the center and our pharmacy, resolving refill, delivery and MAR issues in real time.
• Oversee on-site medication storage, inventory and emergency supplies.
• Educate center staff, participants and caregivers about medications, adherence and ordering processes.
• Support formulary management, quality improvement and CMS PACE and Michigan Board of Pharmacy compliance.

Part-Time Consultant Pharmacist – PACE | Alpena, MI

Part time (one day per week) · On site · $61–$67 per hour

Kalamazoo Long Term Care Pharmacy is looking for a part-time Consultant Pharmacist to work on site at the PACE center we support in Alpena. PACE (Program of All-Inclusive Care for the Elderly) helps older adults stay safely in their homes and communities. Medication management is central to that work.
