Synchrony Pharmacy Columbus in Ohio seeks a Consultant Pharmacist to oversee medication use processes in long-term care facilities. You will review regimens, ensure safety and regulatory compliance, and collaborate with interdisciplinary teams to optimize therapy.
You will provide counseling to residents and caregivers, participate in facility surveys, and travel as needed to support multiple campuses. A Bachelor’s degree in Pharmacy and 3–5 years of pharmacy experience are required.

Serves patients by reviewing medication regimens and identifying medication issues and trends. Ensures that medications are appropriate, effective, safe, and used correctly. Reviews and analyzes medicationrelated problems that may interfere with the goals of therapy. Assists facilities with compliance of regulatory standards, providing expertise with federal and state laws and regulations governing pharmacy services and long-term care.
Education: Bachelor Degree
Experience: 3-5 years
Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours. Must be able to lift at least 50lbs. Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates outside of the health campus
