Job summary

Synchrony Pharmacy, a subsidiary of Trilogy Health Services LLC, is seeking a Consultant Pharmacist for the Pharmacy division to support long-term care facilities. The role emphasizes medication reviews, regulatory compliance, and collaboration within an interdisciplinary team.

Qualified candidates have a Bachelor degree and 3–5 years of pharmacy experience, with an active license and strong communication skills to educate residents, staff, and caregivers.

Qualifications

• Bachelor degree required.
• 3–5 years of experience in pharmacy.
• Licensed pharmacist in good standing with the state.

Responsibilities

• Performs Medication Regimen Reviews (MRR) and provides written reports to facility staff.
• Participates in resident assessment and care planning as part of the interdisciplinary team.
• Assists facilities in developing policies and procedures to ensure regulatory compliance relating to drug handling and administration.
• Coordinates review of controlled substance utilization, reconciliation, and documentation.
• Provides counseling on medications to residents and caregivers and conducts staff in-services as requested.
• Oversees all aspects of pharmacy services related to the medication use process including prescribing, dispensing, and disposal.
