Overview

Synchrony Pharmacy, a subsidiary of Trilogy Health Services LLC, seeks a Consultant Pharmacist for the Pharmacy division.

Responsibilities

Relevant responsibilities include:

• Perform Medication Regimen Reviews (MRR) as required by regulation and provide written reports to applicable facility staff.
• Participate in resident assessment and care planning as a member of the interdisciplinary team.
• Participate on relevant nursing facility committees and assist in developing and implementing policies and procedures.
• Assist the facility in ensuring compliance with federal and state regulations related to drug management.
• Coordinate or perform review of controlled substance utilization, reconciliation, and documentation.
• Provide counseling on medications to residents, caregivers, and staff.
• Oversee all aspects of pharmacy services related to the medication use process.
• Conduct an exit conference when services are completed.
• Assist with issues or questions arising during site surveys.
• Support campus and departmental goals and overall company objectives.
• Comply with all laws, regulations, and ethical conduct standards.
• Perform all duties with a friendly, positive, and team‑oriented approach.
• Maintain required licenses or certifications.
• Travel as needed for facility visits, overnight stays as necessary.

Qualifications

Job Summary Serves patients by reviewing medication regimens and identifying medication issues. Ensures medications are appropriate, effective, safe, and used correctly. Reviews and analyzes medication‑related problems that may interfere with therapy goals. Assists facilities with compliance of regulatory standards, providing expertise with federal and state laws and regulations governing pharmacy services and long‑term care.

Education Bachelor’s Degree

Experience 3-5 years

Physical Requirements Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours. Must be able to lift at least 50 lbs and maintain verbal and written communication with co‑workers, supervisors, residents, family members, visitors, vendors, and business associates outside the health campus.
