Job summary

Consonus Pharmacy is a closed-door long-term care pharmacy serving nursing facilities and assisted living residents. We are hiring a Consultant Pharmacist to join our team that lives in the Eugene or Medford area, focusing on resident care and facility collaboration.

This role requires reviewing medication regimens, educating facility staff, and ensuring compliance with CMS guidelines, with opportunities for advancement and a supportive team environment.

Qualifications

• Pharmacist license in the state(s) of practice.
• Ability to travel throughout a territory, including overnight stays as needed.

Responsibilities

• Review resident medication regimens monthly per patient acuity level and report findings in writing to facility administrator, director of nursing and physician.
• Plan and present in-service education to other pharmacy staff and nursing facility employees as requested.
• Understand and implement all appropriate pharmacy policies & procedures.
• Participate in the development of emergency drug supply kits at each facility in conjunction with the medical director, provider pharmacy and nursing administration.
• Liaison between facility and pharmacy staff.
• Provide ongoing education directly or indirectly to residents preparing for discharge from facilities serviced.
• Knowledge of CMS regulations and interpretive guidelines.
• Attend quality assurance and other appropriate committee meetings as requested by facilities.
• Prepare quarterly quality assurance reports.

Qualifications
