Valley Family Health Care seeks a Population Health Specialist-- Connected Care to support the Quality Department in improving health outcomes for older adults. The role emphasizes proactive outreach, care coordination, patient education, and connection to community resources.

The Care Coordinator - Connected Care works with patients, families, and interdisciplinary teams to address barriers, complete assessments, and advance value-based care goals within VFHC's aging population programs.

Qualificações

• High school diploma or equivalent; healthcare or public health education preferred.
• 2–5 years in healthcare, public health, care coordination, or social services preferred.
• CHW certification or ability to obtain within 6 months.
• Knowledge of Medicare/Medicaid and social determinants of health.
• Strong organization and communication skills; proficient in Excel and EHRs.
• Valid driver's license and ability to travel within the community.

Responsabilidades

• Support quality improvement initiatives and population health strategies.
• Use registries and EHR tools to identify patient needs and barriers to care.
• Review care gaps and coordinate with patients, payors, and care teams to close gaps.
• Complete Connected Care assessments and home, clinic, and community encounters.
• Document interactions and outcomes in the electronic health record.
• Develop relationships with community organizations serving older adults.
