Qualifications

• Minimum of 5 years of professional caregiving experience or equivalent hands-on experience.
• Current California Home Care Aide (HCA) registration in good standing.
• Proof of negative tuberculosis screening or chest X-ray.
• Ability to work independently while following care plans and seeking clarification when needed.
• Strong communication, documentation, customer service, and interpersonal skills.
• Reliable transportation and ability to travel to client homes.
• Ability to use smartphones, tablets, and electronic documentation systems.

Responsibilities

• Deliver personalized 1:1 non-medical care in the client's home.
• Assist with Activities of Daily Living (ADLs) including personal care, bathing, dressing, grooming, toileting.
• Assist with IADLs such as meal planning, light housekeeping, laundry, transportation, shopping, and companionship.
• Complete daily care notes using agency documentation systems.
• Provide support to clients with conditions such as Alzheimer's, dementia, Parkinson's, MS, COPD, CHF.
• Communicate promptly with RN Care Ambassador and care coordination regarding changes or concerns.
• Maintain reliability, punctuality, and consistent attendance.
• Maintain confidentiality and HIPAA compliance.
• Foster respectful relationships with clients, families, and team.
• Work independently in a client's home with sound judgment.
