Job summary

TheKey provides concierge-based in-home care to help clients age with dignity and independence. The Registered Nurse will deliver direct client care, supervise the clinical team, and coordinate services to ensure high-quality outcomes in the New York area.

Maintain an active RN license and meet state regulations while leading care planning, collaborating with physicians and caregivers, and documenting care accurately.

Qualifications

• Current Registered Nurse license in the state of employment with active license in good standing.
• Three to five years experience in health care, elder care, social work, or related industry.
• Current CPR Certification.
• Excellent customer service and conflict resolution skills.
• Computer proficiency and ability to document timely and accurately notes in system.
• Ability to provide care in a variety of settings.

Responsibilities

• Supervise clinical staff and manage client cases.
• Assess physical, psychosocial, spiritual, and environmental status of clients.
• Perform initial client assessment and coordinate plan of care with physicians and caregivers.
• Provide nursing services per plan of care and monitor client status.
• Update plan of care based on changes in client condition and communicate with families.
• Ensure regulatory compliance with state requirements and staff training.
• Lead and supervise the clinical team and conduct caregiver performance evaluations.
• Perform timely assessments of changes in client condition and adjust care as needed.
• Other duties as assigned.
