Qualifications

• Two years of experience in community case management.
• Extensive knowledge of local Fresno/Clovis resources.
• Experience with individuals, community systems, and multidisciplinary teams.
• Current AHA Healthcare Provider CPR card.

Responsibilities

• Coordinate and support Complex Care Patients with the interdisciplinary team.

• Link to community resources and placements to promote health and healing.
