Job summary

Northwestern Medicine is seeking a Community Health Worker II to serve as a trusted bridge between the health system, social services, and the community in Palos Heights, IL. You will help patients access services, provide education, and support social drivers of health through outreach and clinic-based activities.

Responsibilities include working with an interdisciplinary team, conducting screenings, and documenting in the EMR.

Qualifications

• Associates Degree or commensurate experience required.
• Community Health Worker Certification or commitment to complete certification within 2 years.
• Minimum two years of customer service experience in a clinical environment.

Responsibilities

• Act as liaison between health system, social services, and the community.
• Advocate to ensure patient receives necessary care and resources that are clinically and culturally appropriate.
• Provide services in outpatient clinic, in the community, and via telephonic support.
• Document data in the electronic medical record to support the care plan.
• Collaborate with inter-professional team to provide comprehensive patient care.
