Job summary

Premier Health Partners' Atrium Medical Center is seeking a Chart Completion Tech to prepare, scan and validate paper documents, ensuring complete charts for patient records.

Under supervision, you will analyze medical records for documentation deficiencies, handle release of information, and ensure HIPAA compliance while supporting audits and participation guidelines.

Qualifications

  • High school completion or GED required.
  • Course in medical terminology required or currently enrolled.
  • Preferred: health information or related medical field; knowledge of Word and Excel.
  • Minimum 1–3 years of job-related experience.
  • Medical Terminology licensure/certification is preferred.

Responsibilities

  • Preps, scans, and validates paper documents into the optical scanning system to obtain chart completion.
  • Scans and validates loose reports for the department.
  • Analyzes medical records for deficiencies in chart documentation.
  • Responsible for release of information for continuity of care requests.
  • Ensures HIPAA guidelines are followed to reduce liability.
  • Processes requests for government audits and specialized chart reviews.
  • Monitors incomplete charts and follows up with physicians.
  • Follows JC and Conditions of Participation guidelines and all federal/state requirements.
  • Performs other duties as assigned.

Position: Chart Completion Tech

Department: Health Info MGMT Services

Status: SUPPORT (PRN)

Facility: Atrium Medical Center

Under general supervisor, preps, scans, and validates all types of paper documents into optical scanning system to obtain chart completion. Scans & validates all loose reports that are sent to the department. Responsible for analyzing medical records for deficiencies in chart documentation. Responsible for release of information for continuity of care requests. Assures that guidelines of HIPAA are followed to reduce the hospital’s potential liability in releasing records or protected health information. Responsible for processing request for government audit and specialized chart reviews (UVMC only). Responsible for monitoring incomplete charts and follow up with physicians. Responsible for following guidelines of JC and Conditions of Participations and all federal and state. Responsible to perform other duties as assigned and needed.

Medical Office

Preferred experience:

Must have completed a course in medical terminology or be enrolled in a class at the present time.

Knowledge/Skills
  • Ability to read and write in order to review and maintain patient medical record charts, and perform basic mathematic calculations as well as ability to operate standard office equipment as a level normally acquired through completion of high school.
  • Must be able to work independently in a medical office setting.
  • Problem-solving abilities and basic analytical skills necessary.
  • Must possess excellent organizational skills.
  • Ability to type accurately at acceptable levels of speed.
  • Strong verbal and written communication skills.
  • Interpersonal skills required to effectively communicate, understand, interact, and respond to patients, physicians, and other personnel when obtaining and supplying routine factual medical record information via telephone or direct personal contacts on a daily basis.
  • Basic analytical skills necessary in order to determine work priorities, assist in problem resolution and recommend methods of improving workflow.
