Sweetwater Care is seeking a compassionate Speech Therapist to join our skilled nursing facility in Topeka. The role focuses on evaluating and treating speech, language, and swallowing disorders to improve communication and quality of life for residents.

Responsibilities include developing treatment plans, documenting progress, collaborating with a multidisciplinary team, and ensuring care aligns with physician orders and regulatory guidelines.

Qualifications

• Master's degree in Speech Language Pathology from an accredited university.
• Current state licensure in the state practicing.
• Experience in long term care or skilled nursing preferred.
• High school diploma required.
• Strong communication with residents, families and staff.
• Knowledge of local, state and federal guidelines and regulations.

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting and documentation.
• Provides professional treatment for speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plans activities and provides therapeutic services for speech and language disorders.
• Serves as patient advocate and participates in quality assurance and customer service activities.
• Perform patient evaluations and develop treatment plans with short- and long-term goals.
• Carry out patient care plans and progression within the established care plan.
