Lifeline Home Health, part of LHC Group and the Optum family, is hiring a Social Worker - Per Diem to provide medical social services under physician direction and the interdisciplinary team to support patients and families. Qualifications include an MSW from an accredited program, a valid driver's license, CPR certification (preferred), and licensure as a Social Worker in the state

Qualifications

• Master's degree in Social Work from an accredited program (CSWE preferred).
• Current driver's license, vehicle insurance, and access to reliable transportation.

Responsibilities

• Provide medical social services under the direction of a physician and interdisciplinary group.
• Assist in understanding significant social and emotional factors related to the patient's health status and in development of coping mechanisms.
