Job summary

Mercy Health is seeking a Social Worker at St. Charles Hospital to support patients and families through assessment, planning, counseling, and advocacy.

You will coordinate with a multidisciplinary team to ensure holistic patient care and connect individuals with community resources. Requirements include a Bachelor’s degree in social work and active LSW licensure; at least 1 year in social or human services is preferred.

Qualifications

• Bachelor’s degree in social work or related field (required).
• Licensed Social Worker (LSW) in state of practice (required).
• 1 year in social or human services setting (preferred).

Responsibilities

• Evaluates patients’ needs, situations, and support networks to identify goals and develop individualized care plans.
• Provides emotional support, counseling, and education to help patients cope with challenges.
• Manages cases, tracks progress, and connects patients with community resources.
• Advocates for patient resources and addresses systemic barriers.
• Responds to crises and coordinates with the care team.
• Maintains confidential records and stays informed on policies affecting patient populations.
