Job summary

Providence Pointe Healthcare in Paducah, Kentucky is seeking an Assistant Social Worker to support our Social Services Department, promoting emotional, social, and psychosocial well-being for our elders. You will help residents and families navigate transitions and access resources with compassion and integrity.

You will coordinate admissions, discharge planning, care conferences, and advocacy for resident rights, while collaborating with nursing, therapy, and activities teams to deliver

Qualifications

• High school diploma or equivalent.
• Associate or bachelor’s degree in social work, human services, psychology, sociology, or a related field preferred.
• Previous experience in social services, healthcare, or long-term care preferred.
• Strong interpersonal, communication, and organizational skills.
• Ability to approach sensitive situations with empathy, professionalism, and discretion.
• Understanding of resident rights, confidentiality, and person-centered care.
• Ability to work collaboratively with elders, families, and interdisciplinary teams.
• A genuine passion for improving the lives of others.

Responsibilities

• Assist the Social Services Department with admissions, transitions, and discharge planning.
• Provide compassionate support to elders and families, addressing concerns and connecting them with appropriate resources.
• Assist with coordinating care conferences and communicating resident preferences and needs.
• Support resident satisfaction initiatives and help ensure concerns are appropriately documented and addressed.
• Advocate for resident dignity, choice, independence, and individual rights.
• Maintain accurate and confidential documentation in accordance with facility policies.
• Collaborate with nursing, therapy, activities, and other departments to support person-centered care.
• Assist with identifying community resources and coordinating referrals.
• Support social services programming and other departmental responsibilities under appropriate supervision.
