Qualifications

• High school diploma or equivalent required or preferred.
• SSD certificate enrollment within 6 months of employment.
• Ability to communicate with residents, families and staff effectively.
• Interest in social services within long-term care.

Responsibilities

• Assist with admissions processes, facility tours and inquiries.
• Coordinate transportation and procure personal care items as needed.
• Maintain contact with family members and physicians for care plan meetings.
• Document contacts and referrals on the social service log.
• Assist in resolving grievances and theft/loss issues as requested.
• Help residents and families complete various forms and applications.
• Participate in Behavior Management programs as directed.
• Respect residents' rights, dignity and confidentiality at all times.
• Have knowledge of applicable county, state and federal social service laws for long-term care.
