Job summary

Baywood Post-Acute in Campbell, CA is seeking a skilled Speech Therapist (SLP) to provide speech, language, cognitive, and swallowing services to residents in a 45-bed Skilled Nursing Facility. You will evaluate residents, develop plans, and deliver therapy as part of an interdisciplinary team.

The ideal candidate has a Master's in Speech-Language Pathology, CA licensure, and CCC-SLP credentials, with prior SNF or geriatric experience; PRN schedule offers flexibility and meaningful resident care.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current and unrestricted California license as a Speech-Language Pathologist.
• ASHA CCC-SLP certification, as applicable.
• Previous experience in Skilled Nursing, post-acute care, rehabilitation, or geriatric settings preferred.
• Strong clinical assessment, treatment planning, documentation, and communication skills.

Responsibilities

• Conduct comprehensive evaluations of residents with speech, language, cognitive, communication, and swallowing disorders.
• Develop individualized treatment plans based on evaluation findings and goals.
• Provide skilled speech-language pathology services per plan of care.
• Provide dysphagia assessment and treatment as needed.
• Monitor progress and modify treatment plans accordingly.
• Maintain accurate clinical documentation and participate in care plan meetings.
• Collaborate with interdisciplinary team and educate residents, families, and caregivers.
