Job summary

Relient Health in Modesto, CA is seeking a full-time Speech Language Pathologist for a Skilled Nursing Facility. The role emphasizes evaluation, treatment planning, and teamwork to support patients with speech, language, swallowing, and cognitive-communication disorders.

The position offers $55–60 per hour, full medical, dental, vision, and 401(k) benefits, PTO, holidays, and potential sign-on or relocation assistance. CA licensure and CCC-SLP are preferred; CFs under supervision are considered.

Qualifications

• Graduate of an accredited Speech-Language Pathology program.
• Current CA SLP license (or eligibility).
• CCC-SLP preferred, but CFs may be considered when supervision is available.
• Strong interpersonal and documentation skills.

Responsibilities

• Evaluate and treat patients with speech, language, swallowing, and cognitive-communication disorders.
• Develop and implement individualized treatment plans.
• Document patient progress using EMR systems.
• Collaborate with PTs, OTs, nurses, and physicians to deliver coordinated care.
• Educate patients, families, and staff on treatment goals and progress.
