Reliant Care Management Company is seeking a Registered Nurse (RN) for a full-time or part-time role in Brockton, MA. You will assess patients, create care plans, administer medications, monitor vital signs, and coordinate with the care team to ensure quality patient outcomes.

You should hold an active RN license, have strong communication and collaboration skills, and be committed to ongoing professional development and evidence-based practice.

Qualifications

• Current unencumbered RN license to practice in the state.
• Minimum of 1 year of RN experience preferred.
• Excellent communication and interpersonal skills.
• Ability to collaborate with other healthcare professionals.
• Strong problem-solving and critical thinking skills.

Responsabilités

• Assess patients' health needs and develop and implement care plans.
• Administer medications and treatments as prescribed.
• Monitor patients' vital signs and report changes to staff.
• Educate and support patients and families.

• Document progress and outcomes in electronic medical records.
