Job summary

Reliant Care Management, LLC is seeking a Registered Nurse to provide compassionate patient care and collaborate with physicians and the care team in New Philadelphia, Ohio. The RN will assess health needs, administer medications, monitor vital signs, educate patients and families, and participate in care coordination.

Strong communication and problem-solving skills are essential for delivering high-quality care and improving outcomes.

Qualifications

• Current unencumbered state license as a Registered Nurse.
• Minimum of 1 year of experience as a Registered Nurse preferred.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with other healthcare professionals.
• Strong problem-solving and critical thinking skills.

Responsibilities

• Assess patients’ health needs and develop and implement care plans.
• Administer medications and treatments as prescribed.
• Monitor patients’ vital signs and report any changes to the appropriate staff member.
• Communicate with patients and families to provide education and support.
• Collaborate with other healthcare professionals to coordinate care and ensure continuity of treatment.
• Document patients’ progress and outcomes using electronic medical records.
• Stay up-to-date with current research and best practices in nursing.
• Participate in professional development opportunities to maintain and improve skills.
• Other duties as assigned.
