Job summary

Westmoreland Care & Rehabilitation Center is seeking an experienced RN to provide direct nursing care, supervise nursing staff, and coordinate resident care within a 100-bed facility offering short- and long-term services. This role emphasizes teamwork, patient safety, and adherence to care plans.

The position requires an active RN license in Tennessee, familiarity with EHR systems, and strong documentation skills.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the attending physician of any change in condition.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities and regulatory surveys as needed.
