Carbondale Rehabilitation and Healthcare Center in Carbondale, PA is seeking a compassionate Registered Nurse (RN) to join our skilled nursing team on 2nd shift, delivering high-quality resident-centered care.

Qualifications

• Active and unrestricted RN license in PA.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical assessment, attention to detail, and communication.
• Passion for geriatric care and improving residents' lives.
• Current CPR/BLS certification.

Responsibilities

• Assess, plan, implement, and evaluate individualized resident care plans.
• Administer medications and treatments accurately and safely.
• Monitor residents’ conditions and communicate changes to physicians and care team.
• Provide leadership and support to LPNs and CNAs.
• Maintain thorough, timely documentation in compliance with regulations.
• Partner with interdisciplinary teams for coordinated care.
• Respond to emergencies with calm, competent interventions.
• Educate residents and families on health conditions and wellness.
