Qualifications

• Current licensure as a Registered Nurse in Pennsylvania.
• 1+ years of RN experience, preferably in rehab or long-term care.
• Strong communication and interpersonal skills for a fast-paced environment.
• Ability to adapt to changing priorities with professionalism.

Responsibilities

• Provide high-quality, patient-centered care with individualized plans.
• Maintain effective relationships with patients, families and interdisciplinary teams.
• Collaborate with physicians, therapists and other healthcare professionals.
• Monitor and report patient progress and implement improvements.
• Maintain accurate records of care, treatment and medication administration.
• Participate in quality improvement initiatives to reduce readmissions.
