Job summary

West Park Rehabilitation and Nursing Center in Philadelphia, a 176-bed facility near University City, seeks an RN to deliver compassionate, resident-centered care. You will assess residents, develop individualized care plans, and ensure safety and high-quality care.

You will collaborate with physicians, therapists, social workers, and the care team, monitor vitals, administer medications, communicate with families, participate in quality improvement initiatives, and pursue growth opportunities

Qualifications

• Must hold an active RN license in the state of Pennsylvania.
• Provide compassionate, patient-centered care to residents.
• Ability to collaborate with interdisciplinary care team.

Responsibilities

• Deliver high-quality patient care and develop individualized care plans.
• Collaborate with physicians, therapists, and social workers to coordinate care.
